The vast majority of prescriptions are issued correctly but we know that mistakes can sometimes happen and to reduce this happening again, we need to know about them.

Help us to help you by informing us if there are any issues with your prescription medication, providing as much information as possible.

If your medication is provided by an external pharmacy, they will be responsible for rectifying any mistake and we can offer advice and identify any trends and let them know.
