Long Term Conditions

Long Term Condition Reviews

As of April 2022, we have released our new system of working for your Long Term Conditions (LTC), these LTC reviews will be face to face and you will be invited during your birth month.

The LTC reviews include; Asthma, Diabetes, COPD, any Heart condition, Coeliac and more. These invites will be sent via text message to your phone with a link to book an appointment or similarly you can ring the surgery and book one. If you do not have access to a mobile phone with text messages, we are also sending out letters to inform of these reviews.

The reviews roughly last 30minutes and can include one or more conditions. Any queries about these reviews please do not hesitate to call after 2pm on any week day.
